What is needed at Registration
1.) Completed 2012 Player Contract, signed by parent or guardian who is named on the child’s birth certificate or guardianship papers.
2.) Completed 2012 Physical Fitness and Medical History Form, this must be the Pop Warner approved form. Page one (1) is to be completed by the parent/guardian and page two (2) is to be completed by a medical professional.
3.) A passport or wallet sized head shot of the child. The child cannot be wearing a hat, visor, or sunglasses.
4.) Original birth certificate, if a copy has been not verified and stamped by Peace River Conference.
5.) Guardianship papers if applicable.
Exclusive registration is for returning players that
1.) Played the 2011 season AND 2.) Do not have any outstanding balance from the 2011 season.
