Fees

Registration fee is $200.00 per child.
Multiple fundraising and sponsorship options are available. Information will be available at registration.

The following is required at registration:

A. Current and Completed Player Contract Form
B. Current and Complete Medical/Physical Form
C. 2” x 2” Head Shot of Participant
D. Full Report Card for the Previous Academic Year – when available
E. Original birth certificate, if a copy has been not verified and stamped by Peace River Conference.
F. Guardianship papers if applicable.
G. Waiver – if applicable

All paperwork and monies will be due no later than the second Friday in July. If all paperwork and monies are not turned by this date, then the participant will be put on a waiting list. A participant currently on the waiting list, that has fulfilled his obligations, will be placed on the roster.
